In consideration of my child being permitted to participate in Diamond City Sports programs, I, the undersigned parent or legal guardian, acknowledge that participation in athletic activities involves inherent risks of injury, including but not limited to sprains, fractures, concussion, paralysis, and in extreme cases death. I voluntarily assume all such risks on behalf of my child.
I hereby release, waive, and hold harmless Diamond City Sports, Inc., its officers, volunteers, coaches, sponsors, and the owners of any facility used by the league from any and all claims, demands, or causes of action arising out of or related to any loss, damage, or injury sustained by my child while participating in league activities, to the fullest extent permitted by law.
I authorize league staff and volunteers to obtain emergency medical treatment for my child if a parent or guardian cannot be reached, and I accept financial responsibility for any such treatment. I certify that my child is physically able to participate and has no condition that would make participation unsafe, except as I have disclosed to the league in writing.
Players and parents agree to show respect for coaches, officials, opposing teams, and spectators; to display good sportsmanship at all games and practices; and to follow the directions of league staff and volunteers.
